These forms MUST be completed BEFORE you arrive for your appointment. The forms contain your Consent to have your examination and your GDPR forms. Your chiropractor will be unable to see you without these forms. Please check junk files if not received. We are a paperless clinic and you will not be able to complete your forms upon arrival. If you are attending the clinic with a lower back complaint it is advisable to bring some shorts with you for your appointment. A gown will be provided.
